Where are you located?

1800 Howell Mill Road #450, Atlanta, GA 30318.

Our office is located off the Howell Mill exit on I-75 inside the perimeter. You’ll take Exit 252, and our office is immediately off the exit. We are located in the Piedmont West MDVIP building. Following the signs to visitor parking, you will pull into the parking deck to an available open spot. Make sure there are no signs reserving it for a specific reserved use. Be prepared to pay between $5-10 for the parking deck. Current prices will be marked when you drive in. Places to pay prior to getting back in your vehicle are located in the garage near the entrance doors.

How do I schedule an appointment?

You can request an appointment through MyChart. If you’re a new patient and don’t yet have an access code to create your account, or you have misplaced it, you can also call 404-355-4393 to make an appointment.

How do I pay my bill?

Once you create a MedFusion account, you’ll be able to login to pay your bill online. If you’ve forgotten your password, you can also follow the “I forgot my user name and/or password” links to reset it on the sign in page here.

We have 3 patient logins for various purposes: MyChart is for appointments and record-keeping. MedFusion is for billing. CCS is for diabetes equipment supplies.

How do a send my provider a message?

This feature is available within MyChart. During your first appointment, you will setup your username and password, but if you have misplaced it or need access before your next appointment, you can call our office to get further instructions.

How do I reorder diabetic supplies / equipment?

For new patients, call our main number and ask to speak with the CCS administrator.

For existing patients, login to the CCS website.

Are you open on the weekend?

No, we are open Monday through Friday. M-Th 8:30 a.m. to 4:30 p.m. On Fridays we close at 3 p.m. Business functions such as prescription refills cannot be processed after hours.

What are your hours?

Do you treat more than just diabetes?
+

Yes; we provide comprehensive treatment as it relates to endocrinology (glands & hormones). We also frequently hold education courses, and are leaders in cutting-edge diabetes research.

What insurance do you accept?

Please understand that it is your responsibility to know if your provider participates in your plan and that any/all fees not covered by your plan will be your responsibility. The following are insurances that Atlanta Diabetes Associates is NOT contracted with:

  • Coventry One HMO
  • Medicaid
  • Well Care
  • Peach Care
  • Peach State
  • Amerigroup
  • All Marketplace
  • We do not accept insurance discount plans
How should I prepare for my first appointment?

It’s a great idea to complete your New Patient form prior to your appointment. You can print it out, fill it out, and bring it with you. Please visit the New Patient page for the form and helpful other information (such as where to park) that can be helpful to be acquainted with before you arrive.

Is my information kept confidential and secure?

Yes. We maintain HIPAA compliant systems and best practices. Please read our privacy policies for more detail.

How do I participate in research?

Visit the research page for more information on past and future research. You can fill out a form to express your interest in general or for any specific studies you are interested in.

Do you have overnight facilities for research participants?

Yes, we have several overnight rooms for research participants. If you have questions about participating in research, please contact us.

I have a few logins. What does what?

Are you owned by Piedmont Hospital?

Although we’re not hospital owned, we are part of the Piedmont Clinic. If you need a referral to another Piedmont doctor, click here to visit the Piedmont Clinic website.

Do you treat pediatric patients?

We are not listed as a pediatric clinic; however, some of our providers will treat pediatric patients with diabetes.

Can you do labs on site?

We have a full onsite lab where we can run most, if not all, of your lab tests.

Do you offer bilingual support?

Dr. LeBlanc speaks Spanish, along with several other schedulers and support staff at our office. If speaking Spanish is your preference, we will do our best to accommodate those needs.

Do you participate in the MDVIP Wellness Program? Does my doctor really get to know me?
